You must have Adobe Acrobat Reader® version 6 or higher on your computer. If you don't have Acrobat Reader®, or if you need an upgrade, you can use the link below to go to the Adobe web site to get the free download.

 

• CCA membership is a family membership.
• Membership is $20 if you receive the newsletter via email.
• Membership is $30 if you receive the newsletter via US mail.
• Sorry, but you cannot sign up on-line. We need your signature on the waver.
• Please print the form, complete it, and then mail it to the address listed at the bottom of the form.

Download Membership Form

If Adobe Acrobat Reader is on your computer, left click on the download link above and the Membership Form should open on your computer screen. Print the form by clicking the Print icon on the tool bar.

If the Membership Form does not open in your browser, right click on the link and choose Save Target As or Save Link As. Then open and print the form from the location where you saved it on your computer's hard drive.
